In 2026, Michigan changed how people who qualify for both Medicare and Medicaid receive their coverage. The Michigan Department of Health and Human Services introduced MI Coordinated Health, a Highly Integrated Dual Eligible Special Needs Plan (HIDE-SNP). This model integrates medical and pharmacy services under a single health plan with one website and one phone number for support. For MI Coordinated Health members with qualifying chronic conditionsβ€”such as diabetes, cardiovascular disorders, or other chronic conditionsβ€”the plan offers enhanced assistance. If you are chronically ill, you may access benefits that go beyond basic medical care, including help with grocery shopping. Health plans like United Healthcare, and Humana coordinate these long-term services to address the specific needs of the community.

Local Access in Macomb and Wayne Counties

The MI Coordinated Health rollout focuses on select regions, including Macomb and Wayne counties. In these counties, contracted insurers including Aetna Better Health, HAP CareSource, Humana, Meridian Health Plan, Molina Healthcare, Priority Health, AmeriHealth, and UnitedHealthcare have been awarded contracts to serve as HIDE SNP plans. These providers coordinate with your doctor and our team at Family Love Care to provide ongoing support. Because MICH is a highly integrated model, your care coordinator will stay in regular contact to help coordinate your services, excluding those managed by behavioral health carve-outs.

How MICH Supports Independent Living

MI Coordinated Health is designed to help members live as independently as possible in their own homes and communities. Through Long-Term Services and Supports (LTSS), members may access home modifications, personal emergency response systems, home-delivered meals, and respite care for family caregivers.

The program's waiver-based services go beyond basic medical coverage to address the practical needs of daily living. Environmental adaptations, assistive technology, and community transition services help individuals avoid institutional placement and maintain their independence.

Your Dedicated Care Coordinator

Every MI Coordinated Health member is assigned a care coordinator who serves as your single point of contact. This person performs a comprehensive needs assessment, develops your individualized care plan, and coordinates across all your providers β€” from primary care to pharmacy to home care services.

Your care coordinator works directly with Family Love Care to ensure your personal care hours are authorized, your caregiver is properly enrolled, and your services continue without interruption. They help bridge the gap between your health plan and the hands-on care you receive at home.

$0 Out-of-Pocket for Most Services

For dual eligible members enrolled in a HIDE-SNP plan, there is typically $0 cost for covered in-network services, including in-home personal care and medical visits. The primary exception may be a small co-pay for certain Medicare Part D prescription medications. If you have a Patient Pay Amount (PPA) or Pre-Eligibility Medical Expenses (PEME), those amounts remain your responsibility.

To qualify for MI Coordinated Health (MICH), you must be enrolled in both Medicare and full Medicaid. If eligible, you may have a special enrollment period to choose a MICH plan.
