Program Overview

What is the Maryland CFC Program?

The Community First Choice (CFC) program is a Maryland Medicaid state plan option for individuals with disabilities and seniors. It is an entitlement program — meaning there is no waiting list. If you qualify, you are entitled to receive services.

CFC is designed for those needing a higher level of support, typically assistance with two or more Activities of Daily Living (ADLs). Because it is funded by Medicaid, there is no out-of-pocket cost for eligible participants.

Caregiver Qualifications

To become a paid CFC caregiver in Maryland, you must meet the following requirements

Age & Work Eligibility

Must be at least 18 years old and legally authorized to work in the United States.

Background Check

All prospective caregivers must pass a criminal background check before providing services.

Training

State-required training on personal care, safety, and HCBS standards. Family Love Care provides this training at no cost.

Agency Enrollment

Caregivers must work through a licensed Residential Services Agency (RSA) like FLC. Caregivers cannot be paid directly by the state in Maryland.

Can Family Members Be Paid Caregivers?

Yes — participants choose their own caregiver. This is a standard feature of the CFC program, not a separate pathway.

Clinical Requirement

What the Nurse Assessment Covers

A nurse from the Maryland Department of Health will conduct an in-home assessment to determine whether the participant needs help with two or more Activities of Daily Living. The assessment evaluates the participant's daily routines, safety needs, and overall health.

Upon approval, the state creates a Plan of Service with authorized personal care hours based on the participant's assessed needs.

Financial Eligibility

Medically Needy (Spend-Down)

Applicants can apply medical expenses toward an income threshold to qualify for Medicaid. This pathway helps those whose income is slightly above standard limits.

SSI Pathway

Individuals who receive Supplemental Security Income (SSI) automatically qualify for Maryland Medicaid, simplifying the CFC application process.

Higher Income Pathways

Application Process

Medicaid Enrollment

Apply for Maryland Medicaid through the Department of Social Services (DSS). FLC helps ensure all paperwork is accurate and submitted correctly.

An initial in-home assessment by MDH determines eligibility. Then the SPA conducts a detailed assessment to develop the Plan of Service.
